The purpose of this form is to allow citizens the option of filing a minor (not-in-progress) type complaint report to the Fond du Lac Police Department. Minor theft and vandalism complaints where an officer is not needed at the scene would be examples of reports that can be submitted using this form. The incident must have taken place within the City of Fond du Lac. Your submitted report may not be processed immediately. If you think an officer is needed for immediate follow-up, please call (920) 906-5555 to report the incident.
